Have a ride in mind and not sure what to do? The following forms will help;
- Raleigh HOG Ride Planner - Submit this completed form to the Activities Officer with the information about your planned ride / event.
- Charity Ride/Event Request Form - Complete and submit this form to the Charity Ride coordinator at least two (2) weeks prior to the desired calendar posting.

Chapter Event Release Form for Adults - must be signed by all adult participants if
- the individual is not a member of Raleigh HOG
- the individual is a member of Raleigh HOG but has not signed the Chapter Membership Enrollment Form and Release for the year in which the event occurs
- you're unsure whether someone must sign a release or not
- WHEN IN DOUBT, GET A RELEASE FORM SIGNED
- Chapter Event Release Form for Minors - must be signed by parent or legal guardian whenever a minor participates in any chapter event other than a chapter meeting, even if the minor is a member of the chapter. This form must be signed by the parent or legal guardian each time the minor participates.

Minor's Assumption of Risk Acknowledgement - a minor aged 12 years or above must sign this form when participating in any chapter event other than a chapter meeting. This form must be signed by the minor each time the minor participates.
- Note: The Chapter Event Release Form for Minors and the Minor's Assumption of Risk Acknowledgement must be kept as separate pages of one document.
- Incident Report Form - Use when an incident occurs at a chapter event, especially injuries occurring as a result of motorcycling.

Raleigh HOG offers some great riding opportunities with our weekly group chapter rides. We also encourage members to take off on their own and enjoy the great North Carolina highways and byways on their Harley-Davidson motorcycle.
Our Chapter Patch Rides let you enjoy a variety of scenic, fun rides to destinations others have been all the while affording you the ability to earn a patch that is unique to our Raleigh NC Chapter.
